Public Health Registered Nurse Case Manager
Position Summary: The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care, including well-child checkups, immunizations, tuberculosis and other infectious disease screening and diagnostic testing, management of chronic health conditions, and infectious disease case investigations. The role entails training Community Health Aides and Practitioners (CHA/Ps) to competently administer public health testing, examinations, immunizations, and assist with screening, diagnostic testing, treatment, and case investigation as needed.
Responsibilities of the position includes participating in the design, execution, monitoring, quality assurance, and assessment of the Well Child/Immunization and other public health programs. This includes identifying areas of improvement and proposing effective strategies to enhance program performance.
The incumbent is expected to travel to various villages to render services and provide on-site instruction, and supports CHA/Ps in conducting examinations, assisting with testing, and administering immunizations, ensuring adherence to health and safety protocols.
The Case Manager provides education on topics related to immunizations, chronic care, preventive health examinations, and diagnostic and surveillance testing, and is required to communicate effectively with diverse audiences, ensuring the information presented is understood and applied appropriately.
Additionally, the Case Manager assists in providing instruction during Health Aide Training and at Continuing Medical Education (CME) events for CHA/Ps and other clinical staff. Other duties include coordinating with other health care providers, maintaining accurate records of patient interactions, monitoring patient progress, and ensuring patients' health needs are met effectively. The Case Manager must stay updated with the latest public health knowledge and developments, applying this knowledge to improve patient outcomes and overall program effectiveness.
Position Qualifications:
- Minimum Education: Diploma, Associate of Applied Science, or Associate’s Degree in Nursing for RN required, Bachelor of Science in Nursing or Master of Science in Nursing preferred.
- Minimum Experience: 2 years of positive work history as RN or other public health related field. A higher-level degree in related field may be considered in lieu of equivalent professional experience.
- License, Certification, Registration: Must have current RN licensure in State of Alaska. Current BLS, recommend ACLS and PALS
- Equipment/Tools: Must have computer experience. Proficiency in MS Office software including WORD, EXCEL and ACCESS preferred. Must be adept with Cerner and its modules within 3 months of hire. Must be proficient with equipment and tools used in an EPSDT exam within 3 months of hire.
- Specialized Knowledge and Skills: Knowledge and experience with Pediatrics required. Prefer experience in teaching, working in cross-cultural setting and knowledge of the Community Health Aide Program. Excellent verbal and written communication skills. Fluent in English. Fluent in Yugtun preferred
- Supervisory Responsibilities: Must demonstrate the ability to work with and guide student Health Aides and Health Aides in procuring the skills and abilities necessary for success in delivering well-child exams and immunizations.
