Diabetes Prevention Coordinator, Diabetes Prevention & Control Program

Yukon-Kuskokwim Health Corp Bethel, AK (Onsite) Full-Time

Position Summary: In conjunction with the Diabetes Prevention and Control (DP&C) Coordinator and team members, plan, coordinate, and oversee lifestyle coaching, health promotion, and community outreach activities for DP&C. Develops and oversees supplemental grants that support the diabetes program goals. Provides supervision to Diabetes Outreach Specialists I and II, med support technician, and Exercise Physiologist. Assist with development of internal program goals, policies, and procedures. Conducts data collection and management of information systems, including quality control. Provides lifestyle coaching, DSMES, and other diabetes education in-line with educational background. This job requires village travel and formation of partnerships with village tribal councils, school administrators, and other agencies both internal and external to YKHC.

Position Qualifications: This job requires excellent computer and organizational skills. Highly self-motivated individual.

Minimum Education: Bachelor’s Degree in nutrition or exercise science. Preferred Master’s Degree in Public Health, Education, Nutrition, or related health field.

Minimum Experience: Two years of experience in a multi-cultural health care setting providing patient education and case management services. Background in Diabetes preferred.  Preferred experience in outreach, public speaking, case management, and health education activities. Preferred experience with grant reporting and staff supervision.

License, Certification, Registration: Valid CDR and Valid Alaska RD license required or ASEP-EP required. Certified Diabetes Care and Education Specialist (CDCES) certification or ability to obtain within 2 years of hire required. Basic Life Support obtainable within 30 day of hire. 

Equipment/Tools: Work in typical office, hospital, and village clinic setting. Comprehensive knowledge of PC computers. Ability to operate diabetes care medical equipment such as glucometer, ht/weight scale, and other CLIA Waived point-of care testing equipment. Travel throughout the Y-K region in small planes and in sub-arctic conditions is required for successful program implementation. 

Specialized Knowledge and Skills:

REQUIRED:         

Knowledge of health issues, prevention education/ counseling, and strategies for community-based intervention; prevention of chronic diseases and conditions attributed to same (obesity, diabetes and cardiovascular disease); health assessment, prevention, promotion and intervention methods; point of care testing and health education; public relations and written communication skills; strong collaborative and interpersonal skills; educational principles of adult learning theory, behavior change concepts, empowerment models, motivational interviewing techniques, learning styles and teaching strategies; Alaska Native/American Indian community; computer skills to include word processing, data entry and spreadsheets; and grant reporting.  Requires some travel to villages via small plane in sub-arctic conditions, moderate lifting of equipment. May be required to put in occasional long working hours.

PREFFERED:

A professional, working knowledge of community planning, community development and community political structure in rural Alaska; knowledge of social and economic conditions of Alaska Native cultures to enhance the relevance of proposed solutions; bi-lingual (Yup’ik/English); and experience in supervisory role. 

Supervisory Responsibilities: Provide supervision to Outreach Specialists I and II, Exercise Physiologists, and Med Support Technician.
